Delivered by our experienced Registered Nurse, this service supports participants with complex health needs such as wound management, catheter care, health monitoring,
Personal Care Support
Assistance with daily living activities such as bathing, grooming, dressing, toileting, and mobility, enabling participants to live comfortably and independently.
Community Access & Social Participation
Support to participate in community activities, attend social events, and build meaningful connections—fostering inclusion, confidence, and wellbeing.
Domestic Assistance
Help with household tasks including cleaning, laundry, meal preparation, and general home upkeep to maintain a safe and healthy living space.
Occupational Therapy
Tailored support from a qualified Occupational Therapist including functional assessments, assistive technology prescription, skill development.
Capacity Building Supports
Skill-building programs that focus on daily living, communication, budgeting, travel training, and self-management, supporting participants to lead more independent lives.
Supported Independent Living (SIL)
24/7 in-home support for individuals requiring ongoing assistance in a shared or individual accommodation setting, promoting autonomy and community integration.
Transportation Services
Safe and reliable transport services for participants to attend medical appointments, therapies, community activities, education, or other essential engagements.
Medication Management
Medication management refers to a set of practices aimed at ensuring that patients use medications safely, effectively, and appropriately.
Meal Preparation Services
